Resolve overdue appointments and avoid appointments become overdue

(Planned for release Autumn 26 - dependent on EMIS)

Many practices struggle with ensuring patients attend required recall appointments to maintain safe support of their conditions or medications.

This will be most heavily used when first transitioning to use Practice Toolkit.

All patients will be fully and entirely managed via Practice Toolkit within the first 12 months of usage.

Only patients exception coded or where a Test/Review has been missed by a clinician and then selected to be ignored will be relevant for tracking

Which patients will be displayed?

We'll be developing a page accessible from the Recall Monthly Overview to help manage patients who are (based on the the Tests/Reviews required in the Recall Settings);

  1. Already overdue

  2. Predicted to be overdue by the time of their next scheduled appointment

Anomalies:

Filters can be applied to view the true duration since last coding of Tests/Reviews. These filters would then also show;

  • Exception coded - where a patient has been invited numerous times but has not booked required appointments, then exception coded to avoid continuing to invite.

  • Tests/Reviews actively ignored (where the appointment was undertaken, but one or more Tests/Reviews were not coded, but then decided to not be relevant enough to call the patient back in for another appointment)

What will be shown?

To provide the relevant information to decide action;

  • Name/Demographics/Contact info

  • Recalls with overdue Tests/Reviews

  • Grouped into appointment types based on Clinical Roles:

    • Tests/Reviews at risk of duration breach,

    • Reason (Overdue, Exception Coded, Proactively Ignored)

    • last coded dates,

    • next planned appointment

    • duration until next planned appointment (they will show a warning icon if already overdue)

    • Resolution Plan

The Resolution options for patients in this position are;

1) Add the at risk Test/Review into an upcoming appointment of the same type

  • If the same type of Clinical Role appointment is coming up, this is the simplest way to ensure the Test/Review is undertaken quickly with minimum additional effort, and preferable to ignoring

  • For ease of management, we'll break options where this could be one into distinct duration groups for each month ahead.

  • Simple select to add the T&R into the planned appointment or select add all per monthly group (to their respective appointments)

2) Ignore

  • If the breach duration is only a month or 2 depending on clinical capacity the requirement may be ignored.

  • This can be undone at any time, and only avoids you being asked to resolve that breach duration again in future.

3) Add in a one-off appointment(s) to 'bridge' the duration between last coded and next scheduled appointment

  • If there is expected to be a 20 month duration, between last coded and scheduled we'd recommend to add appointment into the month 10 (halfway between) however you can choose an alternative month to avoid known problem months e.g. Summer leave/Vaccine clinics etc.

  • This will create an additional one-off appointment for that specific Patient to undertake the at risk Tests/Reviews.

Where a Test/Review has been added into an appointment, or an entirely additional appointment these are highlighted in the Recalls Monthly Overview page, and can be filtered in or out of the listings.
